Welcome to Alfred State College athletics. There are a few forms you will need to complete before you may be medically cleared to begin your first teams practice.

Please complete the following forms:

  1. The New Athlete Physical Form
  2. Condition of Athletic Participation
  3. HIPAA Information Release
  4. Health Insurance
  5. Sickle Cell Trait Screen

The new athlete physical form must be completed by a physician within six months of your first team practice. The form can be found here: Freshmen/Transfer Physical Form
