Football Camp Reminders
As we approach the start of this football season, all invited football players must have these three very important tasks completed before coming to football camp.
- Review NCAA compliance for eligibility. Check with your Alfred State position coach to verify you're compliant.

If you did NOT complete baseline concussion testing last year follow these directions:

At the bottom of the opening page enter customer code: 4MAEWD8UB2.

Freshman and transfer students:
1. You will need to have a physical within six months of the first day of practice, so after Feb. 18, 2015 if you practice the first day. Forms can be found at the following: http://www.alfredstateathletics.com/athletics/Athletic_Training-Index
The health history portion is filled out by you and brought to your doctor for review. All forms can be faxed to (607) 587-4331 or brought with you during check in.

If you are a freshmen or transfer:
- Click on link to register
- Username is your ASC 800#
- Email is your ASC email address
- Fill in other information as requested
- Click on Register
- You will receive an email from PyraMed with a password. Go back to the log in page, use your 800# and the password that was emailed to you. Do this on a computer as the system will not allow you to log in with a tablet or phone.
- Once logged in, you will see a notice that you have forms to complete. Click on the link and complete the forms. You do not need to print these out as you will sign with electronic signature.
- Note: If you are under 18 years of age, you will need a parent/guardian to also sign these forms.
3. Baseline concussion testing: We are using computer based neurological testing through Immediate Post-Concussion Assessment and Cognitive Testing (ImPACT). You can do this through your home computer before you arrive. The test will take about a half hour during which you will need to be in a quiet area where you will not be disturbed in order to get accurate results. If you are using a laptop computer, use a mouse attachment and not the computers' pad. Go to: www.impacttestonline/testing
